I want to participate in the 2021 season of Goals Soccer LTD. (“GoalsBC”).
ASSUMPTION OF RISKS:
I understand and acknowledge that the sport of Soccer, and events put on by GoalsBC involve risk of physical injury and property damage. I understand that the game of Soccer, and other sports and events put on by GoalsBC are played on fields, whose conditions are variable and unpredictable, with other participants who are unpredictable, in weather and with man-made obstacles or equipment which can change without notice, all of which can create a risk, danger or hazard to me and contribute to the risk of play and result in serious injury to me or my property, and that such risk, danger or hazard can be caused or contributed to by negligence on the part of GoalsBC or other persons or participants, including the failure on the part of GoalsBC or other persons or participants to take reasonable steps to protect me from risks, dangers and hazards of participating in the 2022 season or events of GoalsBC.
I am aware that injuries are common in sports and I freely accept and assume the risks, dangers and hazards of me participating or competing in, spectating, or otherwise attending GoalsBC events and activities during the 2022 season or events and have fully assumed all risk of possible personal injury, death, property damage or loss resulting therein.
I understand that the sport of Soccer, and activities put on by GoalsBC require physical exertion and that I should consult a physician before participating in this or any other strenuous physical activity.
RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT:
In consideration of GoalsBC accepting me as a participant or attendee during the 2022 season and tournaments, I for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge GoalsBC, its owners, and sponsors and all their respective agents, employees, volunteers, officials, servants, contractors, representatives, directors, elected and appointed officials, successors, assigns, participants and attendees (all of whom are collectively referred to as the “Releasees”) of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising by reason of my participation in or attendance at the 2022 season and tournaments of GoalsBC, due to any cause whatsoever, including negligence, breach of contract, or breach of any statutory duty of care including duties owed under the Occupiers Liability Act, R.S.B.C. 1996 c. 337, on the part of the Releasees, including the negligence or failure of the Releasees to take reasonable steps to safeguard or protect me from the risks, dangers and hazards of participating in the 2021 season of GoalsBC referred to above.
I further hereby undertake to hold and save harmless and agree to indemnify the Releasees from and against any and all liability incurred by any or all of them arising as a result of, or in any way connected with my participation in or attendance during the 2021 season of GoalsBC.
I hereby grant and release to GoalsBC, the right to use photographs, audio tapes, and/or videotapes in which I appear in any materials such as videos, films, recordings, still photographs or articles relating to GoalsBC, its programs and services including, but not limited to, brochures, newsletters, or reports, whether broadcast on television, radio, website, social media or any other medium.
By selecting the “I have read, understand, and agree to the Release above” button and hitting the “Complete Registration” button, I acknowledge having read and understood the above RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISK & INDEMNITY AGREEMENT and am aware that I am waiving certain legal rights.
